Music Lesson Contract

 

_______________________ (hereafter “Instructor”) and ___________________________ (hereafter “Student”) hereby agree to the following contract concerning lessons in {instrument}:

 

The Student will have {daily, weekly, etc.} lessons at {location}. Lessons will begin on {date} and end on {date, if desired}.

Each lesson will last {30 minutes} and cost {$20}. Lessons may be extended to {1 hour} at {$30} with the Instructor’s approval. Payment can be made upon services delivered.

The Student will provide his/her own instrument for practice and performance. The Instructor shall provide an additional instrument for instructional purposes.

The Student is responsible for his/her own attendance. If the Student cannot attend a lesson, he/she must notify the Instructor at least {24 hours} in advance. If the student misses 2 or more lessons without prior notice given, the instructor reserves the right to discontinue lessons.
